The first line of treatment for psoriasis includes topical medications applied to the skin. Emollients (Moisturisers): On average, participants with an intermediate monthly household income between €1000 and €2000 found the duration of benefit to be more important compared with those with lower income ( < €1000 per month; β = 0.26; P = .02, Table 4) and viewed the severity of AEs as less important (P = .02 in descriptive analyses, Figure 3A; β = −0.25; P = .03 in regression models, Table 4). Furthermore, participants with intermediate incomes ( €1000- €2000 per month) attached less importance to the treatment delivery method compared with participants with lower income ( β = −0.32; P = .005). Notably, the importance of treatment cost covered by the individual did not differ significantly between income groups (data not shown). When attribute importance was compared between employed and unemployed participants (Figure 3B), employed participants were significantly more concerned about the reversibility of AEs (P = .006; data not shown). Preferences for other attributes were largely independent of the working status.

Topical corticosteroids remain the most widely prescribed medications for plaque psoriasis. These range in strength from weak, over-the-counter steroids such as 1% hydrocortisone to superpotent corticosteroids, such as clobetasol propionate, halobetasol propionate, betamethasone dipropionate in optimised base, and diflorasone diacetate in augmented base (table 2). The Stoughton–Cornell classification ranks the potency of topical corticosteroids on their ability to induce vasoconstriction.3 Topical corticosteroids are available in numerous vehicles including powders, sprays, lotions, solutions, creams, emollient creams, ointments, gels, and tape. Recently, clobetasol propionate and betamethasone valerate have both been introduced in foam vehicles that are cosmetically elegant and should improve compliance. Different vehicles are used on different body sites. For example, the scalp and other hair bearing areas are most easily treated with foams, solutions, and gels. Creams are most useful for daytime use, and ointments, which are often more effective but less appealing cosmetically, can be applied at night. Two possible exceptions are the newer foam vehicles, which have comparable clinical efficacy to ointments.4,5

Since these drugs are proteins, they cannot be administered orally and must be given by injection through the skin or by an intravenous infusion. This treatment is recommended in patients with moderate to severe psoriasis. These drugs target the immune response that leads to the rapid skin cell growth of psoriasis. This seems to have increased their safety profile as well as their effectiveness when compared to older drugs. On the other hand, they are quite expensive costing up to $30,000 a year.
The same treatment for skin psoriasis is beneficial for nail psoriasis. However, since nails grow slow, it may take a while for improvements to be evident. Nail psoriasis can be treated with phototherapy, systemic therapy (medications that spread throughout the body), and steroids (cream or injection). If medications do not improve the condition of nail psoriasis, a doctor may surgically remove the nail.

Psoralens make the skin more sensitive to long-wave UVA (320-400 nm). Methoxsalen (Oxsoralen) is a psoralen that is taken by mouth before UV-A light therapy. More than 85% of patients report relief of disease symptoms with 20-30 treatments. Therapy is usually given two to three times per week on an outpatient basis, with maintenance treatments every two to four weeks to maintain remission. Adverse effects of PUVA therapy include nausea, itching, and burning. Long-term complications include increased risks of sensitivity to the sun, sunburn, skin cancer, skin aging and cataracts. Protective glasses must be worn during and after treatment to prevent cataracts. PUVA therapy is not used for children younger than 12 years of age.

The simplest treatment of psoriasis is daily sun exposure, sea bathing, topical moisturizers, and relaxation. Moisturizers, such as petrolatum jelly, are helpful. Daily application of moisturizing cream to the affected area is inexpensive and successful adjunct to psoriasis treatment. Application immediately after a bath or shower helps to minimize itching and tenderness. Section 3 (2009) of the AAD guideline discusses topical agents and recommends their use adjunctively but not as monotherapy if the disease is extensive or recalcitrant.
