Psoriasis in the mouth is very rare, in contrast to lichen planus, another common papulosquamous disorder that commonly involves both the skin and mouth. When psoriasis involves the oral mucosa (the lining of the mouth), it may be asymptomatic, but it may appear as white or grey-yellow plaques. Fissured tongue is the most common finding in those with oral psoriasis and has been reported to occur in 6.5–20% of people with psoriasis affecting the skin. The microscopic appearance of oral mucosa affected by geographic tongue (migratory stomatitis) is very similar to the appearance of psoriasis. However, modern studies have failed to demonstrate any link between the two conditions.

Dermatology & Plastic Surgery Institute Methotrexate, the oldest systemic therapy for psoriasis, remains one of the most effective treatments for psoriasis and psoriatic arthritis. It has a number of short term side effects including bone marrow toxicity, nausea, aphthous stomatitis, and development of megaloblastic anaemia. Even when properly prescribed, bone marrow toxicity of methotrexate is responsible for several deaths each year.25 Caution must be used in administering other medications to patients on methotrexate because certain drugs result in higher methotrexate levels. Specifically, co-trimoxazole (trimethoprim/sulfamethoxazole) results in increased methotrexate toxicity by several mechanisms.26 Decreases in renal function lower methotrexate clearance thereby increasing toxicity, and thus methotrexate needs to be used with increased caution in the elderly.27 Certain non-steroidal anti-inflammatory drugs also interfere with methotrexate secretion.26 Nausea and megaloblastic anaemia of methotrexate can be avoided by concomitant prescription of folic acid (1–5 mg daily) although this may interfere with the efficacy of methotrexate.28

About one third of people with psoriasis are able to identify a relative, living or dead, with psoriasis. It is estimated that if 1 parent has psoriasis that there is a 15% chance that a child will develop the condition. If both parents have psoriasis this increases to about 75%. Interestingly, if a child develops psoriasis and neither parent is affected there is a 20% chance that a brother or sister will also get psoriasis. This is because the condition is known to skip generations but somewhere there will be a familial link to a relative via either or both parents.

Although psoriasis is unpredictable, there might be triggers that you notice that cause it to flare. It may even be as your child gets older, their psoriasis becomes milder. There is no way of predicting what it is going to do next, or pre-empting the next flare-up, so worrying about it will not change things, your child may pick up on your concerns and anxiety thus causing them distress which could then cause a flare-up. So a laid-back, relaxed approached where possible will help.

Oral Mucosal Clinic Tips for managing psoriasis UV light therapy is typically used in patients with extensive psoriasis. The mechanism of action is unknown, although UVB light reduces DNA synthesis and can induce mild systemic immunosuppression. In PUVA, oral methoxypsoralen, a photosensitizer, is followed by exposure to long-wave UVA light (330 to 360 nm). PUVA has an antiproliferative effect and also helps to normalize keratinocyte differentiation. Doses of light are started low and increased as tolerated.
