What might spark your psoriasis today? Although calcipotriene monotherapy has been shown to be moderately effective in reducing the thickness, scaliness and erythema of psoriatic lesions,8 maximal benefits are achieved when calcipotriene is used in combination with potent topical corticosteroids. A simplified approach for combination therapy is to begin therapy with a “quick-fix” phase, followed by a “transitional phase” and then a “maintenance phase.”9 For example, treatment could be initiated with twice-daily applications of a topical corticosteroid and calcipotriene until the lesions are flat; the maximum duration of this phase is four weeks.

Tuesday, January 19, 2016 Psoriatic plaques that fail to respond to topical therapy may be improved by administration of intralesional corticosteroid injections. Triamcinolone (Kenalog) is often used for this purpose. The agent is injected directly into the dermis of a small, persistent plaque. The concentration is generally 3 to 10 mg per mL, depending on the size, thickness and area of the lesion. The dose of triamcinolone is released gradually over three to four weeks; additional injections may be needed every four to six weeks to improve the response. Disadvantages of intralesional injections include pain during the injection and potential side effects of local atrophy and systemic absorption.
Is It Dandruff or Scalp Psoriasis? Older topical remedies of psoriasis such as anthralin and coal tar are still in use. Because they are somewhat unpleasant to use, especially due to odour, product migration, and local irritation, they are less commonly prescribed than the aforementioned topical medications. Keratolytic preparations such as those containing salicylic acid and emollients are also effective for removing the excess scale that troubles many patients with psoriasis.

When psoriasis appears on the scalp, it’s called scalp psoriasis. Scalp psoriasis is common among people with chronic plaque psoriasis. The American Academy of Dermatology notes it affects the scalp in at least 50 percent of people with chronic plaque psoriasis.

In this study, adherence did not differ significantly between biotechnological treatments studied (alefacept, efalizumab, etanercept).75 Also, Chan et al. verified through a validated self-completion questionnaire that the type of therapy significantly influenced adherence, being markedly superior for biological, oral or phototherapy treatments in relation to topical treatments. Adherence to the biotechnological treatment, quantified at 100%, was superior to all other treatment modalities. There were no differences in adherence levels in relation to the different treatments studied (adalimumab, etanercept and infliximab) and in relation to the type of presentation. Among the sociodemographic factors studied, only tobacco consumption showed an inverse relation to adherence to treatment.74
