The following tips can help people to manage scalp psoriasis: Some people found methotrexate helped their skin. Hannah says it controlled her psoriasis well most of the time she was at university but then “plateaued”. Lisa says she’s also “glad” methotrexate can help with psoriatic arthritis which she thinks she could develop in the future. Zara didn’t find methotrexate made much of a difference to her psoriasis.

Like other immunosuppressant drugs, cyclosporine increases your risk of infection and other health problems, including cancer. Cyclosporine also makes you more susceptible to kidney problems and high blood pressure — the risk increases with higher dosages and long-term therapy.
Combining biologic and phototherapy treatments for moderate-to-severe psoriasis Figure 1 Histology of psoriasis In some cases, psoriasis may involve only the fingernails and toenails, though more commonly nail symptoms will accompany psoriasis and arthritis symptoms. The appearance of the nails may be altered and affected nails may have small pinpoint pits or large yellow-colored separations on the nail plate called "oil spots." Nail psoriasis can be hard to treat but may respond to medications taken for psoriasis or psoriatic arthritis. Treatments include topical steroids applied to the cuticle, steroid injections at the cuticle, or oral medications.

Doctors and Medical Staff Several forms of light therapy have been used to treat psoriasis for hundreds of years. In the 1920s, William Goeckerman combined the use of ultraviolet B (UVB) phototherapy with topical application of tars.14 This inpatient psoriasis regimen, known as the Goeckerman regimen, is still occasionally used, but outpatient regimens using UVB phototherapy with emollients have largely replaced the inpatient regimens.

Psoriasis Treatments Are Getting More Personalized Although doctors choose treatments based on the type and severity of psoriasis and the areas of skin affected, the traditional approach is to start with the mildest treatments — topical creams and ultraviolet light therapy (phototherapy) — in those patients with typical skin lesions (plaques) and then progress to stronger ones only if necessary. Patients with pustular or erythrodermic psoriasis or associated arthritis usually need systemic therapy from the beginning of treatment. The goal is to find the most effective way to slow cell turnover with the fewest possible side effects.
Up Next Breakthrough for Paralyzed Patients In a retrospective study of 48 patients (mean age, 51 yr; 33 women, 15 men), psoralen-UVA (PUVA) therapy was found to be an appropriate treatment alternative for palmoplantar psoriasis, according to Carrascosa et al. It provided similar response rates to systemic treatment and often with increased tolerance and safety. PUVA was found to be effective in 63% of cases of palmoplantar psoriasis. Systemic therapy, however, was required in 47.9% of patients, with acitretin being the drug most often used. Adverse events occurred in 25% of patients, with the most common one being mild erythema (18%).

Psoriasis that has small, salmon-pink colored drops on the skin is guttate psoriasis, affecting about 10% of people with psoriasis. There is usually a fine silver-white buildup (scale) on the drop-like lesion that is finer than the scale in plaque psoriasis. This type of psoriasis if commonly triggered by a streptococcal (bacterial) infection. About two to three weeks following a bout of strep throat, a person's lesions may erupt. This outbreak can go away and may never recur.
If your scalp psoriasis is more severe, you can combine coal tar with other treatments. Talk to your doctor about how best to use coal tar products. You might need to leave the shampoo in your hair for several minutes, several hours, or even overnight.

Torsion dystonia (in ‘Jewish health’) type 2 diabetes The most effective wavelengths of UVB light used for the treatment of psoriasis fall in a very narrow range, 311–313 nm.16,17 This has led to the development of narrowband UVB phototherapy, which is more efficient than broadband phototherapy.16 In the few years that narrowband UVB phototherapy has been used, no increase in cutaneous malignancies has been reported. More experience will be needed to firmly establish the safety of narrowband UVB phototherapy. The excimer laser is a powerful beam of 308 nm light (another form of narrowband ultraviolet light) that has been used successfully to treat localised plaques of psoriasis including those on the palms and soles.18
