Psoriasis is an autoimmune disease that causes red, scaly skin. Classic genome-wide linkage analysis has identified nine loci on different chromosomes associated with psoriasis. They are called psoriasis susceptibility 1 through 9 (PSORS1 through PSORS9). Within those loci are genes on pathways that lead to inflammation. Certain variations (mutations) of those genes are commonly found in psoriasis. Genome-wide association scans have identified other genes that are altered to characteristic variants in psoriasis. Some of these genes express inflammatory signal proteins, which affect cells in the immune system that are also involved in psoriasis. Some of these genes are also involved in other autoimmune diseases.
Better health. Most patients with psoriasis seem to be overweight. Since there is a predisposition for those patients to develop cardiovascular disease and diabetes, it is suggested strongly that they try to maintain a normal body weight. Although evidence is sparse, it has been suggested that slender patients are more likely to respond to treatment.
A-Z Drug Index Peer Exchange 7. Rotational therapy refers to the substitution of one therapy for another after 1 to 2 yr to reduce the adverse effects caused by chronic use and to circumvent disease resistance. Sequential therapy refers to initial use of potent agents (eg, cyclosporine) to quickly gain control followed by use of agents with a better safety profile. Immunomodulatory agents achieve clearance or near clearance of lesions more often than methotrexate or NBUVB.

The most effective wavelengths of UVB light used for the treatment of psoriasis fall in a very narrow range, 311–313 nm.16,17 This has led to the development of narrowband UVB phototherapy, which is more efficient than broadband phototherapy.16 In the few years that narrowband UVB phototherapy has been used, no increase in cutaneous malignancies has been reported. More experience will be needed to firmly establish the safety of narrowband UVB phototherapy. The excimer laser is a powerful beam of 308 nm light (another form of narrowband ultraviolet light) that has been used successfully to treat localised plaques of psoriasis including those on the palms and soles.18
Agminated Heterogeneous Papules on the Neck Topical emollients and creams are agents that sooth and soften the skin. Emollients are rich in fats and oils such as lanolin. They work by moisturizing the skin and protecting it from drying. Regular use of emollients may lessen the need for anti-inflammatories like corticosteroids in psoriasis.

Scopus (97) NSAIDs (Nonsteroidal Anti-Inflammatory Drugs) In addition to sprays and shampoos, the foam or mousse vehicle has become popular with many patients. CP foam has also been found to be highly efficacious in the treatment of scalp psoriasis. One randomized, double-blind study in 188 patients with moderate-to-severe scalp psoriasis compared the use of CP foam 0.05% to both CP solution 0.05% and placebo. Following a 2-week treatment period, 74% of patients treated with CP foam were clear or almost clear of their disease, compared with 63% in the solution treated group.29 Another, more recent, open-label trial on 12 patients with scalp psoriasis used CP 0.05% foam twice daily for 4 weeks. In this study, the PASI score dropped from 5.7 to 1.7 (range: 0.2–4.8, standard deviation [SD]: 1.1) by week 2, and to 1.1 (range: 0.2–2.2, SD: 0.6) by week 4.30 The study is limited by small size and lack of a control group, but demonstrates the significant benefit that can be derived from the use of corticosteroid foams in the treatment of scalp psoriasis.

Psoriasis can be mild, moderate or severe. Your treatment options may depend on how severe your psoriasis is. Severity is based on how much of your body is affected by psoriasis. The entire hand (the palm, fingers and thumb) is equal to about 1 percent of your body surface area.

Videos What are the symptoms of psoriasis? Study Some factors increase the likelihood of developing psoriasis. Recent scientific advances in psoriasis research has resulted in the development of biologic treatment. Unlike other medicines which are made by combining chemicals, biologics are created from human or animal proteins. Biologics are given as injections and treat psoriasis by targeting and blocking overactive immune cells in the body.

Retinoids Disclosure: Medscape Salary Employment Learn the latest In the past, doctors treated psoriasis using a “step-wise approach.” Patients with mild to moderate psoriasis would start with topical therapies and, if they did not respond well to that, move on to other treatments, such as systemic therapy or phototherapy. This approach called for treating people with moderate to severe psoriasis with phototherapy or traditional systemic therapies—drugs such as methotrexate and cyclosporine—before offering them biologic therapies (a type of treatment that works with your immune system).

In a nested analysis of 8731 psoriasis patients and 87,310 controls, the odds ratio of CKD after adjustment for age, sex, cardiovascular disease, diabetes, hypertension, hyperlipidemia, body mass index, use of nonsteroidal anti-inflammatory drugs, and duration of observation was 1.36 in patients with moderate psoriasis and 1.58 in those with severe psoriasis. The relative risk for CKD was highest in younger patients. [23, 24]
Plaque psoriasis may itch or burn, and covers the skin with dry, red lesions that may appear "scaly." The treatment of psoriasis depends on its severity and location, and can be divided into four main categories: topical treatments (creams or ointments applied directly to the skin), ultraviolet light therapy (also called phototherapy, delivered in hospital dermatology departments), systemic medications (medicines that work inside your body) or biologic treatments (medicines based on compounds made by living cells). The use of emollients and soap substitutes also form an integral part of treatment.
