The average age for onset of psoriatic arthritis is 30-40 years of age. Usually, the skin symptoms and signs precede the onset of the arthritis. Clinical improvement was defined as any reduction in the baseline modified PASI score. Our patient experienced clinical improvement as early as treatment 8. Remarkably, a 90% improvement in her modified PASI score was noted at treatment 23 (Figure 2 and Figure 3), representing approximately 11 weeks of treatment (modified PASI score, 0). At this point, she reported significant improvement in her pruritus. Conversely, the untreated side (Figure 2 and Figure 4) remained thick, scaly, and extremely itchy. The entire procedure was extremely well tolerated. A subtle, transient, mild burning sensation occurred only when higher fluences were used. The patient discontinued treatment after her condition cleared. In the follow-up period assessments were performed every 3 weeks, and no further treatments were administered to either the treated or the untreated side of her scalp. The area that was treated with the excimer laser was still free of psoriasis after 10 weeks.

5 Ways to Handle the Cost of Psoriasis Care Several objective measures exist to help quantify the severity of scalp psoriasis. For general plaque psoriasis, common tools include the Psoriasis Area and Severity Index (PASI), the Physician Global Assessment (PGA) or Body Surface Area (BSA) scores.10,11 The most commonly employed tools for scalp psoriasis are modifications of the PASI: the Psoriasis Scalp Severity Index (PSSI), the Scalp-modified PASI, and the scalp PGA (ScPGA). Unlike the PASI, which evaluates disease severity on four distinct body surface areas, these indexes take into account erythema, induration, and desquamation of disease affecting the scalp only. Other instruments exist but are less commonly employed.11,12 There are also patient self-assessment tools that have been developed, including the Dermatology Life Quality Index as well as the Scalpdex, a quality-of-life instrument that addresses symptoms as well as disease impact.12–14 Together, these tools can be used not only to assess symptomatology, but also guide treatment choices and evaluate patient response on a variety of indicators.

The use of these systemic medications, with appropriate safety considerations, is supported by Section 4 (2009) of the AAD guidelines.  Increased risk of infection applies to all systemic immune-suppressing medications, especially when used in combinations. Many of the therapies for psoriasis manipulate the function of the immune system and expose the patient to risk of severe infections while blunting the body’s response. In these patients, findings suggestive of minor infections must be taken seriously, and the risk versus the benefit of continuing the drug in the face of the infection must be weighed.
