Apple cider vinegar can be particularly helpful to soothe the itching and burning associated with scalp psoriasis. It contains natural germ-killing properties and can be soothing for the scalp. It should not be applied to broken or cracked skin. For a gentler treatment, the vinegar can be diluted with equal parts water. If it burns when applied, stop using it.

With chronic use, acitretin can be associated with the development of calcification of ligaments and tendons. Although these developments are usually asymptomatic, pain near ligaments and tendons in patients on acitretin warrants radiological examination to determine if the drug should be discontinued. Retinoids can benefit psoriatic arthritis in some patients.32
Intralesional corticosteroids have been applied in practice, although specific studies evaluating the effects of this treatment regimen on scalp psoriasis are lacking. Anecdotal reports of their use exist, and in 2009, the US National Psoriasis Foundation recommended intralesional corticosteroids as second-line treatment for scalp psoriasis.5

As well as itching, the reddish, scaly lesions are visible and are often embarrassing. 'Topical' treatments (drugs applied to the skin, e.g. as creams) are usually tried first, but applying them to the scalp is difficult because of the hair. There are a number of topical drugs in use, such as corticosteroids (also known as steroids), vitamin D, tar-based preparations, tacrolimus, dithranol or salicylic acid. Some topical corticosteroids have more potency than others so are categorised into four levels of strength: mild, moderate, high and very high. As psoriasis remains a long-term condition, it is of great importance to know which of the drugs work best, what kind of side effects they may have and how likely they are to occur.

In the past, doctors treated psoriasis using a “step-wise approach.” Patients with mild to moderate psoriasis would start with topical therapies and, if they did not respond well to that, move on to other treatments, such as systemic therapy or phototherapy. This approach called for treating people with moderate to severe psoriasis with phototherapy or traditional systemic therapies—drugs such as methotrexate and cyclosporine—before offering them biologic therapies (a type of treatment that works with your immune system).
If you have a more severe case of scalp psoriasis, you may need to try different treatment plans before you find the one that works for you. If crusting of the scalp along with scaling occurs and/or the lymph nodes in your neck are enlarged, indicating a yeast infection, your doctor may prescribe antimicrobial treatment.

Some young people report the onset of psoriasis following an infection, particularly strep throat. One-third to one-half of all young people with psoriasis may experience a flare-up two to six weeks after an earache, strep throat, bronchitis, tonsillitis or a respiratory infection.
