There are many choices of biologics approved for the treatment of psoriasis: Acitretin is prescribed in doses of 10–50 mg daily. At doses of 10–25 mg in combination with UVB or PUVA phototherapy, acitretin is highly effective in clearing the condition in most patients with minimal side effects.31 Higher doses are associated with more adverse reactions but limited improvement in psoriasis. With chronic use, acitretin can be associated with the development of calcification of ligaments and tendons. Although these developments are usually asymptomatic, pain near ligaments and tendons in patients on acitretin warrants radiological examination to determine if the drug should be discontinued. Retinoids can benefit psoriatic arthritis in some patients.32 Add systemic agents as needed (methotrexate, retinoid, cyclosporine) ^ Jump up to: a b Benedek TG (June 2013). Talamonti M Data on psoriasis treatment in children are limited, with very few clinical trials and lack of evidence-based treatment guidelines. Therapeutic decisions are essentially based on the individual's experience of the clinician, having as main concern the safety of the child. Regarding adolescents, the possibility of active sexual life should be considered and, as such, categories of risk in pregnancy should be considered. 51 Classic genome-wide linkage analysis has identified nine loci on different chromosomes associated with psoriasis. They are called psoriasis susceptibility 1 through 9 (PSORS1 through PSORS9). Within those loci are genes on pathways that lead to inflammation. Certain variations (mutations) of those genes are commonly found in psoriasis.[30] Genome-wide association scans have identified other genes that are altered to characteristic variants in psoriasis. Some of these genes express inflammatory signal proteins, which affect cells in the immune system that are also involved in psoriasis. Some of these genes are also involved in other autoimmune diseases.[30] No comments have so far been submitted. Managing Type 1 Diabetes While the exact cause of psoriasis is still not completely understood, the immune system and genetics are important in its development. Environmental factors can also play a role, for example an infection or emotional stress may trigger an episode of psoriasis, or worsen an existing outbreak. Obesity, especially abdominal obesity, is considered a low-grade pro-inflammatory state, which increases the risk of cardiovascular disease and metabolic syndrome.
