Pigmentation disorders The efficacy and long-term safety of calcipotriol has been evaluated in a number of studies. One randomized, double-blind study of 49 patients evaluated the efficacy of twice daily calcipotriol solution (50 mcg/mL) over a 4-week treatment period. The study reported that 60% of patients treated with calcipotriol solution were clear or demonstrated marked improvement in their symptoms as compared with only 17% in the placebo group. The total sign score for scalp psoriasis (ie, sum of the scores for redness, induration, and scale) decreased by 49% in the treatment group compared with 19% in the placebo group (P=0.005). Patients’ self-reports also demonstrated that calcipotriol was superior in reducing scalp flaking and itch.37

Anthralin FRIENDSHIPS In some cases, psoriasis may involve only the fingernails and toenails, though more commonly nail symptoms will accompany psoriasis and arthritis symptoms. The appearance of the nails may be altered and affected nails may have small pinpoint pits or large yellow-colored separations on the nail plate called "oil spots." Nail psoriasis can be hard to treat but may respond to medications taken for psoriasis or psoriatic arthritis. Treatments include topical steroids applied to the cuticle, steroid injections at the cuticle, or oral medications.

Scalp psoriasis is a common skin disorder that makes raised, reddish, often scaly patches. Scalp psoriasis can affect your whole scalp, or just pop up as one patch. This type of psoriasis can even spread to the forehead, the back of the neck, or behind the ears. Scalp psoriasis symptoms may include only slight, fine scaling. Moderate to severe scalp psoriasis symptoms may include dandruff-like flaking, dry scalp, and hair loss. Scalp psoriasis does not directly cause hair loss, but stress and excess scratching or picking of the scalp may result in hair loss.

If you’re feeling embarrassed or self-conscious about your scalp psoriasis, a hat, hair accessories, or a new haircut can help you conceal your lesions and flaking. When choosing a hat or accessories, however, be sure to avoid hats that are tight or can cause friction, as they may worsen your symptoms. If you’re getting a haircut, be sure to let your stylist know about your condition, so he can take extra care when selecting hair products and cutting, washing, and styling your hair.
Drug choice can be complicated, and your physician will help in selecting the best option. In some patients. it may be possible to predict drug efficacy on the basis of a prospective patient's genetics. It appears that the presence of the HLA-Cw6 gene is correlated with a beneficial response to ustekinumab.
A screening tool for PsA called the ‘Psoriasis Epidemiology Screening Tool’ (PEST) is available to help general practitioners and dermatologists identify patients for further evaluation by a rheumatologist. It is recommended that patients with psoriasis who do not have a diagnosis of PsA complete a PEST questionnaire annually and are referred on to a rheumatologist where necessary i.e. those patients scoring 3 or more out of 5 (please see diagram).
