Coal tar has been used as a treatment for psoriasis for several decades due to its anti-proliferative and anti-inflammatory actions, and its potent anti-pruritic properties.22 Although crude coal tar is the most effective tar available for the treatment of psoriasis, the application of crude coal tar to the scalp presents a challenge. As such, coal tar solution or liquor carbonis detergens 5%–20% is frequently recommended, formulated as a lotion, or added to a corticosteroid preparation. Of note, the use of coal tar has largely fallen out of favor to newer topical medications. Tar’s malodor, adverse effects (hair staining and drying), and carcinogenicity limit its use, although newer studies challenge these findings.23,24 The use of coal tar in cosmetic preparations has been banned in Canada and the European Union over concerns regarding carcinogenicity.

SPAM PROTECTION - DO NOT FILL THIS FIELD IN The best way to keep it away is to treat and control your psoriasis. To keep scales in check before they can flake and crack, try a scale softening product (keratolytic). Look for these active ingredients: salicylic acid, lactic acid, urea, or phenol. Moisturize to lock in moisture, too.
Light therapy (phototherapy) involves using natural or artificial ultraviolet (UV) light to treat the psoriasis symptoms. A doctor may recommend brief daily exposure to the sun, but too much sunlight can make psoriasis worse. More aggressive forms of light therapy include using controlled doses of UV light on the affected skin, laser therapy, and therapies that combine UV light with medicines and topical treatments.

Donate Online The odds of having hypertension are 1.58 times higher in people with psoriasis than those without the condition; these odds are even higher with severe cases of psoriasis. A similar association was noted in people who have psoriatic arthritis—the odds of having hypertension were found to be 2.07 times greater when compared to odds of the general population. The link between psoriasis and hypertension is not currently understood. Mechanisms hypothesized to be involved in this relationship include the following: dysregulation of the renin–angiotensin system, elevated levels of endothelin 1 in the blood, and increased oxidative stress. The incidence of the heart rhythm abnormality atrial fibrillation is 1.31 times higher in people with mild psoriasis and 1.63 times higher in people with severe psoriasis. There may be a slightly increased risk of stroke associated with psoriasis, especially in severe cases. Treating high levels of cholesterol with statins has been associated with decreased psoriasis severity, as measured by PASI score, and has also been associated with improvements in other cardiovascular disease risk factors such as markers of inflammation. These cardioprotective effects are attributed to ability of statins to improve blood lipid profile and because of their anti-inflammatory effects. Statin use in those with psoriasis and hyperlipidemia was associated with decreased levels of high-sensitivity C-reactive protein and TNFα as well as decreased activity of the immune protein LFA-1. Compared to individuals without psoriasis, those affected by psoriasis are more likely to satisfy the criteria for metabolic syndrome.

Outcome In a significant proportion of patients, therapy was changed in the first year of treatment. Rates of persistence with biotechnology therapy in daily clinical practice at the end of one year ranged from 22% to 85% in different studies.32-34 In the Italian observational study OSCAR, at 29 months of treatment, 81% of patients maintained their treatment with etanercept, 59% with adalimumab and 62% with infliximab.35 However, in the Danish registry DERMBIO, the highest median retention rate was obtained with ustekinumab, followed by adalimumab and infliximab (59 months) and, finally, by etanercept (30 months). 36

Amyotrophic lateral sclerosis Most people with moderate to severe psoriasis will benefit from a combination of treatments. This type of therapy uses more than one of the treatment types to reduce symptoms. Some people may use the same treatment their entire lives. Others may need to change treatments occasionally if their skin stops responding to what they’re using.
