Although severity of disease influences a person’s psychological response and the quality of life impact they experience, different people respond to psoriasis in different ways. Some people with mild psoriasis are seriously affected by psychological complaints, while others with severe psoriasis have few psychological complaints.

This may also affect their ability to maintain social relationships. In one study, 11% of people with psoriasis reported not wanting to have children for fear that they would inherit the condition.

Immunosuppressants can be used for severe psoriasis. Cyclosporine is a commonly used immunosuppressant. It should be limited to courses of several months (rarely, up to 1 yr) and alternated with other therapies. Its effect on the kidneys and potential long-term effects on the immune system preclude more liberal use. Other immunosuppressants (eg, hydroxyurea, 6-thioguanine, mycophenolate mofetil) have narrow safety margins and are reserved for severe, recalcitrant psoriasis.
Psoriasis is known to have a negative impact on the quality of life of both the affected person and the individual's family members. Depending on the severity and location of outbreaks, individuals may experience significant physical discomfort and some disability. Itching and pain can interfere with basic functions, such as self-care and sleep. Participation in sporting activities, certain occupations, and caring for family members can become difficult activities for those with plaques located on their hands and feet. Plaques on the scalp can be particularly embarrassing, as flaky plaque in the hair can be mistaken for dandruff.
Scalp psoriasis can be extremely uncomfortable, but with the right treatments and mental and emotional support, you can better manage your condition. Work with your healthcare provider or dermatologist to determine the best treatment options for your case and lifestyle.

With psoriasis, the immune system becomes overactive causing these immune cells to move to the top layer of skin (the epidermis) and act as if they are healing a wound. During this process, these hyperactive immune cells cause your blood cells to dilate or widen (making the patches very red) and more immune cells move towards the surface of the skin. Psoriasis appears as a small bump, a papule, surmounted by scale. When these papules coalesce, a plaque is formed that is often covered by thick layers of horny scale. When this scale is shed, it appears as dandruff, which can be quite unsightly. Scratching these plaques, either because of itching or because of the impulse to remove it, is a very poor idea because of what is called the Koebner phenomenon (also known as the Koebner response or isomorphic response).
