Professionalism Award Take a Stand Against Step Therapy Psoriasis is a chronic, inflammatory disease which can affect the skin, joints and nails. The causes of psoriasis are not fully understood and are the subject of ongoing research. In psoriasis, the immune system is mistakenly activated, which leads to overproduction of skin cells. Skin cells build up too rapidly on the surface of the skin, forming raised, red, scaly patches (called plaques). These plaques are often itchy and sometimes painful. Psoriasis lesions commonly appear on the scalp, but they can occur anywhere on the body.

Attention Deficit Disorder If you’re feeling embarrassed or self-conscious about your scalp psoriasis, a hat, hair accessories, or a new haircut can help you conceal your lesions and flaking. When choosing a hat or accessories, however, be sure to avoid hats that are tight or can cause friction, as they may worsen your symptoms. If you’re getting a haircut, be sure to let your stylist know about your condition, so he can take extra care when selecting hair products and cutting, washing, and styling your hair.

What can reduce psoriasis flare-ups? (video) Psoriasis is an immune-mediated, genetic disease manifesting in the skin or joints or both. A diverse team of clinicians with a range of expertise is often needed to treat the disease. Psoriasis provides many challenges including high prevalence, chronicity, disfiguration, disability, and associated comorbidity. Understanding the role of immune function in psoriasis and the interplay between the innate and adaptive immune system has helped to manage this complex disease, which affects patients far beyond the skin. In this Seminar, we highlight the clinical diversity of psoriasis and associated comorbid diseases. We describe recent developments in psoriasis epidemiology, pathogenesis, and genetics to better understand present trends in psoriasis management. Our key objective is to raise awareness of the complexity of this multifaceted disease, the potential of state-of-the-art therapeutic approaches, and the need for early diagnosis and comprehensive management of patients with psoriasis.

Further information Psoriasis has a genetic basis and can be inherited. Some people carry genes that make them more likely to develop psoriasis. Just because a person has genes that would make him more likely to have psoriasis doesn't mean he will have the disease. About one-third of people with psoriasis have at least one family member with the disease. Certain factors trigger psoriasis to flare up in those who have the genes.

However, for people with chronic plaque psoriasis receiving long-term treatment with corticosteroids, there remains a lack of evidence about the risk of skin dermal atrophy. Further research is required to inform long-term maintenance treatment and provide appropriate safety data.

Why it works: “These medications work by calming the immune response and telling the skin cells to grow and turn over at a regular rate,” Daveluy says. Vitamin D analogues calm your overactive immune response, which is causing the excess skin cell production that leads to psoriasis. Vitamin D analogues (or forms of activated vitamin D), including calcipotriene and Rocaltrol (calcitriol), can be part of liquid scalp treatments or may be combined with a topical steroid. “The combination product is particularly effective and useful for mild to severe scalp psoriasis,” he says.

Find a doctor The most important consideration in treating scalp psoriasis is getting an effective medication into the skin. Both the hair and any scale covering the disease act as an impediment to treatment. Removal of the scale in a nontraumatic fashion is very important. This can be accomplished by shampooing frequently. Using tar, selenium, or salicylic acid-containing shampoos can be helpful. This may have to be done at least twice a day initially if the scale is sufficiently thick. Gently rubbing off the scale with the fingertips and not the nails is important.
