Why it works: “We actually don’t know exactly how coal tar works, but it’s one of the oldest treatments for psoriasis,” Daveluy says. “In mild scalp psoriasis, it may be the only treatment necessary to keep it under control.”
(9) Getty Images Infectious Bacteria Hibernate, Evade Antibiotics Skin cells, which are made deep in the skin, normally take about a month to rise to the surface, where they die and are sloughed off. When psoriasis triggers T cells to attack healthy skin, the immune system responds by sending more blood to the area and making more skin cells and more white blood cells. This forces skin cells to rise to the surface in a few days instead of a month. The dead skin and white blood cells can't be shed quickly enough, and they build up on the surface of the skin as thick, red patches. As the skin cells die, they form silvery scales that eventually flake off.

For psoriasis of the scalp, a 2016 review found dual therapy (vitamin D analogues and topical corticosteroids) or corticosteroid monotherapy to be more effective and safer than topical vitamin D analogues alone. Due to their similar safety profiles and minimal benefit of dual therapy over monotherapy, corticosteroid monotherapy appears to be an acceptable treatment for short-term treatment.
Vitamin D analogues are used to help control overactive skin cell production by binding to the vitamin D receptors on the skin cells. Topical vitamin D analogues such as calcipotriene (Dovonex, Sorilux) and calcitriol (Vectical) are effective at slowing the growth of the skin cells and can be used with emollients and topical corticosteroids applied to the skin. Calcipotriene with betamethasone (Taclonex, Enstilar) is a vitamin D analogue that is already combined with a corticosteroid.
