Specifically, co-trimoxazole (trimethoprim/sulfamethoxazole) results in increased methotrexate toxicity by several mechanisms.26 Decreases in renal function lower methotrexate clearance thereby increasing toxicity, and thus methotrexate needs to be used with increased caution in the elderly.27 Certain non-steroidal anti-inflammatory drugs also interfere with methotrexate secretion.26 Nausea and megaloblastic anaemia of methotrexate can be avoided by concomitant prescription of folic acid (1–5 mg daily) although this may interfere with the efficacy of methotrexate.28

LIGHT THERAPY Dandruff and scalp psoriasis can be sometimes difficult to distinguish. Both are common conditions that affect the scalp and they may share similar symptoms (e.g. itch, redness and scaling). The following table may help you to distinguish between these two conditions by visually examining your and dandruff. However, telling the difference can be difficult. If you’re in doubt about whether your dandruff may indicate scalp psoriasis scaling, talk to your doctor.

Yeast onychomycosis is caused by Candida and may be the most common cause of fungal fingernail.
Immunomodulators (tacrolimus and pimecrolimus) have also been used with some limited success in mild psoriasis. These have the advantage of not causing skin thinning. They may have other potential side effects, including skin infections and possible malignancies (cancers). The exact association of these immunomodulator creams and cancer is controversial.

Since psoriasis is incurable, the selection of treatment plans must consider the long-term outlook. Treatment options depend on the extent and severity as well as the emotional response to the disease. They include topical agents (drugs applied to the skin), phototherapy (controlled exposure to ultraviolet light), and systemic agents (orally, intravenously, or percutaneously administered agents). All of these treatments may be used alone or in combination with one another. Psoriasis in children younger than 15 years of age is rare; therefore, the following review is confined to adult options.
