What Causes Pustular Psoriasis? Combining biologic and phototherapy treatments for moderate-to-severe psoriasis Causes of psoriasis & video
Philipp S Biologic treatment has revolutionized the care of patients with moderate-to-severe psoriasis but is far more costly, overall, than other treatments. However, patient co-payments for biologics are often significantly less than the start-up costs for home phototherapy.6 The increasing cost to manage psoriasis can largely be attributed to the use of biologics in a relatively small percentage of patients.7 The total economic burden of psoriasis was estimated to be $112 billion in the United States for 2013.8

"If I'm treating a patient who also has psoriatic arthritis, then I wouldn't just use topicals. I'd want to make sure we have some systemic agent on it. One of the scary things about psoriatic arthritis is that it's irreversible, so I'd want to make sure we at least get in the way of some of that inflammation so we don't have any permanent joint damage," Dr. Buka adds.

Psoriasis treatments reduce inflammation and clear the skin. Treatments can be divided into three main types: topical treatments, light therapy and systemic medications. Scholars believe psoriasis to have been included among the various skin conditions called tzaraath (translated as leprosy) in the Hebrew Bible, a condition imposed as a punishment for slander. The patient was deemed "impure" (see tumah and taharah) during their afflicted phase and is ultimately treated by the kohen. However, it is more likely that this confusion arose from the use of the same Greek term for both conditions. The Greeks used the term lepra (λεπρα) for scaly skin conditions. They used the term psora to describe itchy skin conditions. It became known as Willan's lepra in the late 18th century when English dermatologists Robert Willan and Thomas Bateman differentiated it from other skin diseases. Leprosy, they said, is distinguished by the regular, circular form of patches, while psoriasis is always irregular. Willan identified two categories: leprosa graecorum and psora leprosa.

Psoriasis is an immune-mediated, genetic disease manifesting in the skin or joints or both. A diverse team of clinicians with a range of expertise is often needed to treat the disease. Psoriasis provides many challenges including high prevalence, chronicity, disfiguration, disability, and associated comorbidity. Understanding the role of immune function in psoriasis and the interplay between the innate and adaptive immune system has helped to manage this complex disease, which affects patients far beyond the skin. In this Seminar, we highlight the clinical diversity of psoriasis and associated comorbid diseases. We describe recent developments in psoriasis epidemiology, pathogenesis, and genetics to better understand present trends in psoriasis management. Our key objective is to raise awareness of the complexity of this multifaceted disease, the potential of state-of-the-art therapeutic approaches, and the need for early diagnosis and comprehensive management of patients with psoriasis.
The effectiveness of treatments varies depending on your symptoms. You may have to work with your health care provider to find the best treatment, or combination of treatments, for your psoriasis. No matter what treatment you use, the key is keeping up with the medicine on a regular basis. This may mean putting on a cream twice a day or taking a pill daily, even when you don’t have flare-ups. Some people find it helpful to put an alarm on their cell phone as a reminder. Just remember, psoriasis treatment works best when used regularly.

This is supported by histologic examination and immunohistochemical staining of psoriatic plaques revealing large populations of T cells within the psoriasis lesions. One report calculated that a patient with 20% body surface area affected with psoriasis lesions has around 8 billion blood circulating T cells compared with approximately 20 billion T cells located in the dermis and epidermis of psoriasis plaques.
