Key findings in the affected skin of patients with psoriasis include vascular engorgement due to superficial blood vessel dilation and altered epidermal cell cycle. Epidermal hyperplasia leads to an accelerated cell turnover rate (from 23 d to 3-5 d), leading to improper cell maturation. Approximately 7.5 million adults in the United States are affected by this disorder; about 17% have moderate-to-severe disease.1–3 Treatment options for moderate-to-severe psoriasis include phototherapy, oral agents (including cyclosporine or methotrexate), and injectable biologics. Home phototherapy, perhaps the most cost-effective treatment in the long run, is safe and effective, and incurs relatively few costs: those of acquiring the equipment, initially, and of bulb replacement every 3 to 6 years.4 Acquisition of home units is often hindered by qualification protocols defined by each insurance company.5 Even when home units are covered, patients’ co-payment requirements may discourage their use, leading many to choose biologics instead.5 There’s no cure for psoriasis of the scalp, but you have options. Group B received placebo twice weekly for 12 weeks followed by 12 weeks of etanercept 50 mg twice weekly (n=62). The primary end point was percentage change in PSSI at week 12.49 The etanercept group (Group A) had a mean PSSI improvement at week 12 of 86.8%, compared to only 20.4% in the placebo group (Group B) (P<0.0001). By week 24, Group A had a mean PSSI improvement of 90.6%, which was not significantly different from Group B, which had 79.1% improvement subsequent to initiating etanercept therapy at week 13. Eighty-six percent of patients in Group A achieved a PSSI-75 at week 12, compared with only 11% in Group B (P<0.0001). However, at week 24, the PSSI-75 rates were similar at 86% and 72% for Group A and B, respectively. There was also improvement noted in scalp surface area involvement, with a mean percentage improvement at week 12 in Group A of 84.6% versus only 14.9% in Group B. At week 24, Group A maintained this benefit at 84.8%, while Group B showed improvement at 75.4%. Changes in overall PASI improvement were consistent with the PSSI improvements and time to improvement was similar; treatment also provided a high level of patient satisfaction.49 Is one of the most commonly used oral treatments for psoriasis Service Search Frequent Questions Biological treatments Science Newsletter: Subscribe Neonatal and Pediatric Medicine Open Access Journal The most common form of the disease is plaque psoriasis. The main topical treatments are corticosteroids (cortisone creams, gels, liquids, sprays, or ointments), vitamin D-3 derivatives, coal tar, anthralin, or retinoids. These drugs may lose potency over time so often they are rotated or combined. Ask you doctor before combining medications, as some drugs should not be combined. Treatments are often combined and rotated because your psoriasis may become less responsive to medications after repeated use. Systemic treatments are not commonly used only for scalp psoriasis but may be used if psoriasis is present elsewhere on the body or if psoriasis is moderate to severe.
