Some biologics are to be administered by self-injections for home use while others are given by intravenous infusions in the doctor's office. Biologics have some screening requirements such as a tuberculosis screening test (TB skin test or PPD test) and other labs prior to starting therapy. As with any drug, side effects are possible with all biologic drugs. Common potential side effects include mild local injection-site reactions (redness and tenderness). There is concern of serious infections and potential malignancy with nearly all biologic drugs. Precautions include patients with known or suspected hepatitis B infection, active tuberculosis, and possibly HIV/AIDS. As a general consideration, these drugs may not be an ideal choice for patients with a history of cancer and patients actively undergoing cancer therapy. In particular, there may be an increased association of lymphoma in patients taking a biologic. Biopsy Treatment for psoriasis usually helps to keep the condition under control. Clinical features Systemic retinoids (eg, acitretin, isotretinoin) may be effective for severe and recalcitrant cases of psoriasis vulgaris, pustular psoriasis (in which isotretinoin may be preferred), and hyperkeratotic palmoplantar psoriasis. Because of the teratogenic potential and long-term retention of acitretin in the body, women who use it must not be pregnant and should be warned against becoming pregnant for at least 2 yr after treatment ends. Pregnancy restrictions also apply to isotretinoin, but the agent is not retained in the body beyond 1 mo. Long-term treatment may cause diffuse idiopathic skeletal hyperostosis (DISH). guttate psoriasis – found mostly in children Practice Management Psoriasis is characterized by an abnormally excessive and rapid growth of the epidermal layer of the skin.[45] Abnormal production of skin cells (especially during wound repair) and an overabundance of skin cells result from the sequence of pathological events in psoriasis.[18] Skin cells are replaced every 3–5 days in psoriasis rather than the usual 28–30 days.[46] These changes are believed to stem from the premature maturation of keratinocytes induced by an inflammatory cascade in the dermis involving dendritic cells, macrophages, and T cells (three subtypes of white blood cells).[12][37] These immune cells move from the dermis to the epidermis and secrete inflammatory chemical signals (cytokines) such as interleukin-36γ, tumor necrosis factor-α, interleukin-1β, interleukin-6, and interleukin-22.[30][47] These secreted inflammatory signals are believed to stimulate keratinocytes to proliferate.[30] One hypothesis is that psoriasis involves a defect in regulatory T cells, and in the regulatory cytokine interleukin-10.[30] Global Burden of Diseases , the Collaborative Association Study of Psoriasis (CASP), the Genetic Analysis of Psoriasis Consortium, the Psoriasis Association Genetics Extension, the Wellcome Trust Case Control Consortium 2 Meta-analysis of psoriasis, cardiovascular disease, and associated risk factors. Some studies compared vitamin D products directly with potent or very potent corticosteroids. These products had similar effects when applied to the body, but corticosteroids worked better than vitamin D for scalp psoriasis. Treatment that combined vitamin D with a corticosteroid was more effective than vitamin D alone and more effective than the topical corticosteroid alone. Vitamin D products generally performed better than coal tar, but studies found conflicting results when comparing vitamin D with dithranol.
