The treatment of psoriasis depends on its severity and location, and can be divided into four main categories: topical treatments (creams or ointments applied directly to the skin), ultraviolet light therapy (also called phototherapy, delivered in hospital dermatology departments), systemic medications (medicines that work inside your body) or biologic treatments (medicines based on compounds made by living cells). The use of emollients and soap substitutes also form an integral part of treatment. The incidence of diabetes is 27% higher in people affected by psoriasis than in those without the condition.[102] Severe psoriasis may be even more strongly associated with the development of diabetes than mild psoriasis.[102] Younger people with psoriasis may also be at increased risk for developing diabetes.[101][103] Individuals with psoriasis or psoriatic arthritis have a slightly higher risk of heart disease and heart attacks when compared to the general population. Cardiovascular disease risk appeared to be correlated with the severity of psoriasis and its duration. There is no strong evidence to suggest that psoriasis is associated with an increased risk of death from cardiovascular events. Methotrexate may provide a degree of protection for the heart.[36][101] Law & Medicine Jump up ^ Farahnik, B; Beroukhim, K; Zhu, TH; Abrouk, M; Nakamura, M; Singh, R; Lee, K; Bhutani, T; Koo, J (March 2016). Eighty-eight percent of patients using BMV 0.12% foam had complete or near complete resolution of scaling compared to only 66% of patients using standard therapies of corticosteroid or calcipotriol (P<0.001).32 Patients also reported that the foam was a more convenient formulation to use as opposed to standard therapies.32 Another trial on 79 patients showed that once daily application of BMV 0.12% foam was as effective as twice daily application for 4 weeks.33 This is important given that once daily efficacious treatment has a much higher adherence rate as compared to twice daily treatments.34 Methotrexate can cause nausea and may affect the production of blood cells. In a population-based cross-sectional study of 9035 psoriasis patients and 90,350 matched controls without psoriasis, those with more extensive psoriatic skin disease were at greater risk for major medical comorbidities, including heart and blood vessel disease, chronic lung disease, diabetes, kidney disease, joint problems, and other health conditions. [17, 18] Overall, the risk for any other type of serious illness was 11% higher for people with mild psoriasis, 15% higher for patients with moderate psoriasis, and 35% higher for those with severe psoriasis. [17, 18] Guttate psoriasis is characterized by red dots and spots spread throughout the skin. The dots and spots are not as thick as plaques in plaque psoriasis. Although severity of disease influences a person’s psychological response and the quality of life impact they experience, different people respond to psoriasis in different ways. Some people with mild psoriasis are seriously affected by psychological complaints, while others with severe psoriasis have few psychological complaints. Many of the clinical features of psoriasis are explained by the large production of such mediators. Interestingly, elevated levels of TNF-α specifically are found to correlate with flares of psoriasis.
