Psoriasis is a chronic inflammatory disorder characterized by well-demarcated, erythematous plaques with thick silver-white scale.1 Affecting 1%–2% of the North American population, psoriasis is both a physically and psychologically debilitating disease, impacting on quality of life similar to cancer, diabetes, and depression.2 Although psoriasis can present at any age, major peaks of incidence occur around 20 and 60 years of age.3 Approximately 80% of patients with psoriasis have mild-to-moderate disease, with up to 5% of their body surface area affected.4 Importantly, psoriasis tends to run a chronic course with remissions and exacerbations. Successful management is dependent on a number of factors including patient education, choice of therapeutics, and adherence to treatment course.

Light therapy may be used alone or in combination with medications. Ultraviolet light lessens the growth of plaques, redness, swelling and itching. Methoxsalen (Oxsoralen-Ultra) can be used with UV light also, called PUVA, and is effective in treating larger areas of widespread psoriasis. Laser light (UVB) is also used and can directly target psoriasis and avoid the surrounding skin.

The idea is to change the anti-psoriasis drugs every six to 24 months in order to minimize the toxicity of one medication. Depending on the medications selected, this proposal can be an option. An exception to this proposal is the use of the newer biologic medications as described below. An individual who has been using strong topical steroids over large areas of their body for prolonged periods may benefit from stopping the steroids for a while and rotating onto a different therapy.
