Severe psoriasis not responding to the above may need tablets such as methotrexate, acitretin and cyclosporin. Unfortunately, oral medications can cause side effects such as bone marrow damage, liver damage and kidney damage, and requires regular blood tests to detect these side effects. New injectable biologic agents which are safe and effective are also available for patients who do not respond to conventional therapy.
What Causes Pustular Psoriasis? Michael P Schön Plaque psoriasis is the most common form with plaques covered in silvery scales. The plaques can occur anywhere on the body, including the soft tissue of your mouth. The most common area of chronic plaque psoriasis on the body is a person’s scalp and/or lower back.
In terms of clearance, as assessed by the IGA, steroids were better than vitamin D (RR 1.82; 95% CI 1.52 to 2.18; four studies, 2180 participants, NNTB = 8; 95% CI 7 to 11; moderate quality evidence). Statistically, the two-compound combination was superior to steroid monotherapy, however the additional benefit was small (RR 1.22; 95% CI 1.08 to 1.36; four studies, 2474 participants, NNTB = 17; 95% CI 11 to 41; moderate quality evidence). The two-compound combination was more effective than vitamin D alone (RR 2.28; 95% CI 1.87 to 2.78; four studies, 2008 participants, NNTB = 6; 95% CI 5 to 7; high quality evidence).

Industry ( 1364) Several objective measures exist to help quantify the severity of scalp psoriasis. For general plaque psoriasis, common tools include the Psoriasis Area and Severity Index (PASI), the Physician Global Assessment (PGA) or Body Surface Area (BSA) scores.10,11 The most commonly employed tools for scalp psoriasis are modifications of the PASI: the Psoriasis Scalp Severity Index (PSSI), the Scalp-modified PASI, and the scalp PGA (ScPGA). Unlike the PASI, which evaluates disease severity on four distinct body surface areas, these indexes take into account erythema, induration, and desquamation of disease affecting the scalp only. Other instruments exist but are less commonly employed.11,12 There are also patient self-assessment tools that have been developed, including the Dermatology Life Quality Index as well as the Scalpdex, a quality-of-life instrument that addresses symptoms as well as disease impact.12–14 Together, these tools can be used not only to assess symptomatology, but also guide treatment choices and evaluate patient response on a variety of indicators.
Scalp, Hair and Nails Neuroscience and Psychiatry The scalp is one of the most common sites to be affected by psoriasis, and sometimes it is the only area of involvement. It usually extends to, or just beyond the hairline and commonly occurs behind the ears. Scalp psoriasis may appear in the form of fine white flakes (similar to dandruff) or the scale may become thickened, with an appearance like cradle cap.
What does Psoriasis look like? Inverse psoriasis causes smooth, raw-looking patches of red skin that feel sore. The patches develop in places where skin is touching skin, such as the armpits, buttocks, upper eyelids, groin and genitals, or under a woman's breasts. Being overweight increases a person's risk of developing inverse psoriasis.
Psoriasis is a genetically programmed inflammatory disease that primarily affects the skin in about 3% of individuals in the United States. Psoriasis is characterized by skin cells that multiply up to 10 times faster than normal. When these cells reach the surface and die, raised, red plaques covered with white scales form. Psoriasis begins as a small scaling papule. When multiple papules coalesce, they form scaling plaques. These plaques tend to occur in the scalp, elbows, and knees.

The condition can make a trip to the hair salon a nerve-wracking experience. In the future, combining the determination of serum drug concentrations and the detection of anti-drug antibodies may facilitate therapeutic optimization.42,45-46 Carrascosa et al. describe a model of therapeutic decision based on these assays, considering that, in the case of secondary inefficacy and optimal serum drug concentrations, the treatment should be replaced by a drug with a different mechanism of action.45-46 In cases of suboptimal serum concentrations, anti-drug antibodies should be dosed, with two scenarios depending on the presence or absence of anti-drug antibodies. If antibodies are positive, the drug should be replaced with another, but the mechanism of action may be maintained. In the case of negative antibodies, the dose of the drug may be increased in an attempt to improve the clinical response.45-46 However, the value of immunological tests for detecting anti-drug antibodies in daily clinical practice remains controversial.42
