Psoriasis is a chronic, inflammatory disease which can affect the skin, joints and nails. The causes of psoriasis are not fully understood and are the subject of ongoing research. In psoriasis, the immune system is mistakenly activated, which leads to overproduction of skin cells. Skin cells build up too rapidly on the surface of the skin, forming raised, red, scaly patches (called plaques). These plaques are often itchy and sometimes painful. Psoriasis lesions commonly appear on the scalp, but they can occur anywhere on the body.
Because psoriasis is a chronic disease with no cure, patients may need to use treatments for a long time. Many therapies approved by FDA have been evaluated for extended time periods.
The most common form of psoriasis that affects about 80% of all sufferers is psoriasis vulgaris ("vulgaris" means common). It is also referred to as plaque psoriasis because of the well-defined areas of raised red skin that characterize this form. These raised red plaques have a flaky, silver-white buildup on top called scale, made up of dead skin cells. The scale loosens and sheds frequently.

Skin Gallery The rate of psoriasis in HIV-positive individuals is comparable to that of HIV-negative individuals, however, psoriasis tends to be more severe in people infected with HIV. A much higher rate of psoriatic arthritis occurs in HIV-positive individuals with psoriasis than in those without the infection. The immune response in those infected with HIV is typically characterized by cellular signals from Th2 subset of CD4+ helper T cells, whereas the immune response in psoriasis vulgaris is characterized by a pattern of cellular signals typical of Th1 subset of CD4+ helper T cells and Th17 helper T cells. It is hypothesized that the diminished CD4+-T cell presence causes an overactivation of CD8+-T cells, which are responsible for the exacerbation of psoriasis in HIV-positive people. Psoriasis in those with HIV/AIDS is often severe and may be untreatable with conventional therapy. In those with long-term, well-controlled psoriasis, new HIV infection can trigger a severe flare of psoriasis and/or psoriatic arthritis.

These are creams or ointments containing synthetic forms of vitamin D. Their purpose is to slow down the growth of skin cells. Calcipotriene (Dovonex) is a prescription cream or solution containing a vitamin D analogue that may be used alone to treat mild to moderate psoriasis or in combination with other topical medications or phototherapy. This treatment can irritate the skin. Calcitriol (Rocaltrol) may be equally effective and possibly less irritating than Calcipotriene.
