When I returned to see the doctor, I wanted to give him the good news that he had cured me, but the truth was his ointment couldn't control the 'dermatitis'. I eked out each prescription for as long as I could, to put off having to go back for a morale-sapping inspection.

Courtesy of Calcipotriene cream is useful in psoriasis because of its effect on calcium metabolism. The advantage of calcitriol is that it is not known to thin the skin like topical steroids. There is a newer combination preparation of calcipotriene and a topical steroid called Taclonex. Not all patients may respond to calcipotriene. Prolonged use of these types of medications on more than 20% of the skin surface can produce an abnormal rise in body calcium levels.

Treatment of psoriasis in pregnant women and during breast-feeding should be approached with caution. Although the disease typically improves during pregnancy and gets worse 4 to 6 weeks postpartum, the worsening of psoriasis in 23% of women during pregnancy has also been reported. Methotrexate and acitretin are contraindicated in pregnancy and are not recommended during breast-feeding. Cyclosporine may be considered as an alternative for use in pregnant women and should be avoided during breast-feeding due to the risk of immunosuppression of the baby. Regarding biotechnology drugs, recommendations are for discontinuation for varying times prior to conception, depending on the elimination half-life, due to the absence of controlled studies in pregnant women. However, information has been accumulating regarding its use, mainly in patients with inflammatory arthritis or inflammatory bowel disease. There are concerns on immunosuppression of fetuses exposed to biotechnology drugs at the end of the second and third trimesters of pregnancy, especially for monoclonal IgG antibodies. In addition, administration of live vaccines to newborns exposed to biotechnology drugs during the end of the second and third trimesters should be postponed until 6 to 7 months after birth. Breast-feeding is not recommended during therapy with biotechnology drugs, but may be considered reasonable in the future, as the amounts of drug found in milk are negligible. Decision of biotechnology therapy during pregnancy should be made on a case-by-case basis and shared with the patient.50

Individual Sign In Sugiura K Although there are fewer trials to support their benefit, other very potent topical corticosteroids (eg, betamethasone dipropionate) and medium to potent topical corticosteroids (eg, betamethasone valerate [BMV]) were reported to have a similar beneficial effect to CP.18 The medium potency 0.12% BMV foam (Luxiq®; Prestium Pharma, Newtown, PA, USA) was shown to be highly efficacious in the treatment of scalp psoriasis, and to be significantly more efficacious than BMV 0.1% lotion with enhanced penetration.31 In a placebo-controlled study including 179 patients with moderate-to-severe scalp psoriasis, 72% of patients using BMV foam achieved clear or almost clear status, compared to only 47% using BMV 0.1% lotion and 21% of patients using placebo foam or lotion.31 A cross-over, multi-center study comparing the foam to standard therapies had 210 patients complete the treatment period. Eighty-eight percent of patients using BMV 0.12% foam had complete or near complete resolution of scaling compared to only 66% of patients using standard therapies of corticosteroid or calcipotriol (P<0.001).32 Patients also reported that the foam was a more convenient formulation to use as opposed to standard therapies.32 Another trial on 79 patients showed that once daily application of BMV 0.12% foam was as effective as twice daily application for 4 weeks.33 This is important given that once daily efficacious treatment has a much higher adherence rate as compared to twice daily treatments.34

It's pretty safe to say that scalp problems of any nature (itchiness, flaking, and the like), are highly annoying to anyone suffering from them. But scalp psoriasis — a common condition characterized by thick, red scaly patches of skin that can cover the entire scalp and be chronic — is arguably the most painful and inconvenient of them all. Scalp psoriasis treatment often comes in the form of oils, steroid solutions, and even ointments, but basic care for it starts in the shower with the shampoo you use. Those battling scalp psoriasis want to aim for a formula that simultaneously sloughs away flakes as it soothes irritation. New York City-based dermatologist and founder of Smarter Skin Dermatology, Sejal Shah, says that you should look for a shampoo comprised of ingredients such as coal tar, which is a dark liquid that's a by-product of the production of coke and coal gas from coal and is used in medications to treat psoriasis and seborrheic dermatitis. It may sound scary, but dermatologists say it significantly helps to slow down the rapid growth of skin cells while reducing inflammation. Ahead, find Shah's favorite shampoo picks for scalp psoriasis, as well as other expert recommendations.
LinkedIn Combination therapy with a vitamin D analog (calcipotriol and calcipotriene) or a retinoid such as tazarotene and a topical corticosteroid is more effective than therapy with either agent alone. Oatmeal baths may be helpful for itching.

OMICS Journal of Radiology Open Access Journal Dermatology Daily Cells that normally lose their nuclei in the stratum granulosum retain their nuclei, a condition known as parakeratosis. In addition to parakeratosis, affected epidermal cells fail to release adequate levels of lipids, which normally cement adhesions of corneocytes. Subsequently, poorly adherent stratum corneum is formed leading to the flaking, scaly presentation of psoriasis lesions, the surface of which often resembles silver scales.
Higher potency topicals are reserved for thicker, tough-to-treat areas like elbows or knees. Topical treatments work best on mild and smaller areas of psoriasis.
