Some authors also include in this designation the forms of psoriasis in sensitive areas, such as psoriasis of the face and inverse psoriasis. In addition to their location, these forms of psoriasis are characterized by the greater ineffectiveness of topical treatment and may be classified as moderate to severe psoriasis, even if BSA≤10 and PASI≤10.3 There are few controlled clinical trials evaluating the efficacy and safety of systemic therapy (conventional or biological) in these locations. In general, the existing evidence comes from clinical trials in which patients are subanalysed with this type of involvement.47

Dithranol, one of the oldest medications used in the treatment of psoriasis,25 works by inducing free radical production, thus inhibiting several aspects in the pathogenesis of psoriasis.26,27 Dithranol is commonly prescribed as a 0.1%–3% cream, starting at a low concentration and increasing as tolerated. Dithranol is generally indicated in patients with resistant disease, as treatments are generally provided at specialized centers or inpatient departments. This is another treatment that has been replaced by newer, more cosmetically appealing options.

Moisturizers, especially with therapeutic concentrations of salicylic acid, lactic acid, urea, and glycolic acid may be helpful in psoriasis. These moisturizers are available as prescription and nonprescription forms. These help reduce the scales that impede the movement of topical medications into the deeper layers of the skin. Some available preparations include salicylic acid (Salex) and lactic acid (AmLactin, Lac-Hydrin). These may be used one to three times a day on the body. Other bland moisturizers, including Vaseline and Crisco vegetable shortening, may also be helpful in at least reducing the dry appearance of psoriasis.

ANTHRALIN Staying Safe A 49-year-old white woman presented with a 20-year history of scalp psoriasis. Over the years, she had tried numerous topical treatments, such as 0.05% clobetasol propionate solution with 0.005% calcipotriene solution for 3 months, as well as various tar- and salicylic acid–based shampoos, with no success. She stated that the itching associated with her scalp psoriasis had worsened over the past year, often keeping her awake at night. She had abandoned medical treatment for 6 months. She was otherwise in good health and was not taking any medications. There was no family history of skin disease. Physical examination revealed thick, hyperkeratotic, scaly, erythematous, confluent plaques almost completely covering the scalp, including the postauricular area, clinically consistent with scalp psoriasis.

The anti-TNF receptor fusion protein, etanercept, has the most clinical trial data supporting its use in scalp psoriasis. Etanercept was given in an open-label trial of 2,555 patients to compare continuous versus interrupted therapy.48 PGA was the primary end point and ScPGA was reported as a secondary end point. All patients (continuous therapy and interrupted therapy groups) were treated with etanercept 50 mg twice weekly for the first 12 weeks of the study. Subsequently, the continuous therapy cohort continued with 50 mg once weekly for an additional 12 weeks; subjects in the interrupted therapy cohort held therapy (if subject achieved PGA ≤2) until they lost response, at which point etanercept was restarted at week 16 or 20 and was continued up until the end of the study at week 24.48
