Your details Scalp psoriasis is one of the scalp conditions that you should never ignore. “Unfortunately a lot of the topical treatments are thick, gooey ointments that need to be rubbed on the scalp, and this can be a non-starter for some,” Dr. Farber says. The main ingredients in these products are tar or salicylic acid. “I recommend that people massage the lotion or foam into their scalp at night and then put on a shower cap to sleep,” he says. “The key thing is to be using something that fits with routine and lifestyle,” he says. “The good news is that once the scalp psoriasis is under control you can decrease frequency to every other day or twice a week, Dr. Bagel adds. If OTC topical products don’t do the trick, your dermatologist may prescribe Dovonex (a form of vitamin D that slows skin cell growth and removes scales), Taclonex (vitamin D-steroid combo), Tazorac (a topical retinoid that slows skin cell growth) or Anthralin, which reduces the rapid growth of skin cells.

Staying healthy overall. Bowel Cancer Some topical treatments can be messy, and some can cause staining, and so this needs consideration when wearing good clothing, and indeed sitting on household furnishings. Always read product/patient instruction leaflets before use. Set aside some old clothes, night bedding, etc if you are to be using messy treatments.

Who treats it? Association of psoriasis with stroke and myocardial infarction: meta-analysis of cohort studies. Other than age-appropriate screening for cardiovascular disease, long-term monitoring is generally treatment specific (eg, skin cancer in phototherapy patients, liver disease in methotrexate patients, tuberculosis exposure in patients on biologic medications).
The diagnosis of psoriasis is typically made by obtaining information from the physical examination of the skin, medical history, and relevant family health history.
Psoriasis is not curable. Explore the Academy Because it can irritate, redden, and dry the skin, you should test products containing coal tar in a small area at first.

Moisturizers and emollients such as mineral oil, petroleum jelly, calcipotriol, and decubal (an oil-in-water emollient) were found to increase the clearance of psoriatic plaques. Some emollients have been shown to be even more effective at clearing psoriatic plaques when combined with phototherapy. However, certain emollients have no impact on psoriasis plaque clearance or may even decrease the clearance achieved with phototherapy, eg. the emollient salicylic acid is structurally similar to para-aminobenzoic acid (PABA), commonly found in sunscreen, and is known to interfere with phototherapy in psoriasis. Coconut oil, when used as an emollient in psoriasis, has been found to decrease plaque clearance with phototherapy. Medicated creams and ointments applied directly to psoriatic plaques can help reduce inflammation, remove built-up scale, reduce skin turnover, and clear affected skin of plaques. Ointment and creams containing coal tar, dithranol, corticosteroids (i.e. desoximetasone), fluocinonide, vitamin D3 analogs (for example, calcipotriol), and retinoids are routinely used. Truth in advertising state laws Videos & Tools (News) Acral Pigmented Lesions More Common on Darker Skin Despite the vast array of treatment options available for psoriasis, psoriasis of the scalp remains a difficult treatment area. The presence of hair not only impacts the application and penetration of medications to affected areas, but also strongly influences treatment adherence. Patients commonly complain of the greasy effect of medications in this area, and difficulty removing products from their hair. Currently, topical medications are recommended as a first line of treatment in mild-to-moderate psoriasis but can also be used concomitantly with phototherapy, systemic or biological therapies for moderate-to-severe psoriasis. Topical corticosteroids, with or without the addition of the vitamin D analog calcipotriol, are the gold standard for the treatment of scalp psoriasis. Importantly, in recent years, there have been a number of new formulations introduced to the market (eg, foams, shampoos, and sprays) that enhance cosmetic acceptability and adherence. In severe or recalcitrant cases, systemic treatment should be considered.
Biologic drugs are a type of systemic therapy. A biologic is a protein-based drug that is made from living cells grown in a laboratory. Biologics target the specific T cells that are associated with psoriasis.

Classic genome-wide linkage analysis has identified nine loci on different chromosomes associated with psoriasis. They are called psoriasis susceptibility 1 through 9 (PSORS1 through PSORS9). Within those loci are genes on pathways that lead to inflammation. Certain variations (mutations) of those genes are commonly found in psoriasis. Genome-wide association scans have identified other genes that are altered to characteristic variants in psoriasis. Some of these genes express inflammatory signal proteins, which affect cells in the immune system that are also involved in psoriasis. Some of these genes are also involved in other autoimmune diseases.

Psoriasis undergoes “koebnerization,” manifested by the appearance of lesions at sites of cutaneous trauma. Itching is the most common symptom,4 and extensive scratching can often lead to superimposed lichen simplex chronicus. In some patients, burning and pain may be the only symptoms.

A number of genes have been associated with different types of psoriasis. If psoriasis runs in your family, infections and certain medications (lithium, beta blockers, non-steroidal anti-inflammatory medications and antimalarial medication) can trigger the onset of psoriasis or cause it to flare up. Skin injury and smoking can also make certain types of psoriasis worse.
