Because topical treatment regimens can be arduous and typically do not result in sustained clearance, patient expectations should be ascertained prior to initiation of therapy. Topical corticosteroids often can be used as monotherapy in patients with mild psoriasis.3 Topical vitamin D analogues and retinoids also can be effective; however, combined use of these agents with topical steroids should be considered to increase efficacy, and combination formulations can be prescribed to simplify application and improve adherence. Möller S Apremilast and dimethyl fumarate are newer medicines that help to reduce inflammation. They are taken as daily tablets. These medicines are only recommended for use if you have severe psoriasis that has not responded to other treatments, such as biological treatments. Use: Psoralens are taken by mouth 45-60 minutes prior to UVA exposure. Occasionally, psoralens have been applied to the skin in creams, lotions, or bath soaks. This requires close medical monitoring due to the propensity to produce burns. Treatments frequency should not be shorter than 48 hours. Breast Cancer: Current Research Open Access Journal PUVA is a special treatment using a photosensitizing drug and timed artificial-light exposure composed of wavelengths of ultraviolet light in the UVA spectrum. The photosensitizing drug in PUVA is called psoralen. Both the psoralen and the UVA light must be administered within one hour of each other for a response to occur. These treatments are usually given in a physician's office two to three times per week. Several weeks of PUVA is usually required before seeing significant results. The light exposure time is gradually increased during each subsequent treatment. Psoralens may be given orally as a pill or topically as a bath or lotion. After a short incubation period, the skin is exposed to a special wavelength of ultraviolet light called UVA. Patients using PUVA are generally sun sensitive and must avoid sun exposure for a period of time after PUVA. Common side effects with PUVA include burning, aging of the skin, increased brown spots called lentigines, and an increased risk of skin cancer, including melanoma. The relative increase in skin cancer risk with PUVA treatment is controversial. PUVA treatments need to be closely monitored by a physician and discontinued when a maximum number of treatments have been reached. When we performed subgroup analyses, we found that preferences are, in part, dependent on sociodemographic and socioeconomic characteristics. Older patients ( ≥65 years) appear to care less about the probability of improvement compared with younger individuals. In most other studies20,21 using time trade-off, willingness to pay, and visual analog scales to assess health state utilities in psoriasis, utilities were largely independent of age. However, psoriasis-related impairment may be perceived differently among different age groups. For example, it has been reported5,12,40,41 that older patients experience psoriasis more physically than mentally compared with younger patients. Correspondingly, younger patients with psoriasis were shown22 to have greater willingness to pay for social comfort and emotional health than were older patients. Our descriptive analyses also showed the tendencies that women care more about the probability of improvement than men and that singles place greater value on this attribute compared with individuals living with a partner (Figure 2A and C). Although differences did not achieve statistical significance in our sample, it seems likely that women and singles attach special importance to appealing appearance, not the least because of expectations from society and from potential partners. Coal tar is most effective when it is used in combination with other agents, especially ultraviolet B light. Like calcipotriene, coal tar is effective when it is combined with topical corticosteroids. Coal tar shampoo can be used in combination with a corticosteroid scalp solution for the treatment of psoriasis on the scalp. Treating psoriasis may be time consuming and on average those using topical treatments like skin creams report spending 26 minutes per day applying the treatments. The impact on physical and mental functioning associated with the condition is comparable to that associated with arthritis, depression, cancer and heart disease. Diet & Weight Loss Severe psoriasis is diagnosed if the psoriasis covers over 10 pecent of the body, or sometimes if the psoriasis causes significant disruption to daily life. PUVA therapy (psoralen + UVA) As the name suggests, this psoriasis treatment uses natural or artificial ultraviolet light. The simplest form of phototherapy involves exposing your skin to controlled amounts of natural sunlight. Other forms of light therapy include the use of artificial ultraviolet A (UVA) or ultraviolet B (UVB) light. In any form, the light treatment is only effective if the patient gets short-exposure – burns and skin damage can worsen psoriasis symptoms. It's usually administered two or three times a week until the skin improves, and then maintenance may require only weekly sessions. Narrow band UVB phototherapy may cause more-severe and longer lasting burns, however. Jump up ^ Roberson ED, Bowcock AM (September 2010). "Psoriasis genetics: breaking the barrier".
